Home Care  
  • Personal Care - A Personal Care Aide helps the client with personal hygiene.

  • Homemaking - An Aide will help the client to remain at home by performing light housekeeping functions.

  • Respite Care - Provides relief for the primary caregiver by caring for the client in their home.

  • Companion Care - A Companion supports the client through scheduled in-home visits.

  • Information & Referral Assistance - Trained staff will answer any written or verbal requests for information about available services and resources throughout the region. (1-888-597-2751)  Provide intake for the Butler County Elderly Services Program.

  • Benefits/Family Advocate - A Benefits Advocate help the client with completion of financial payments, applications and benefit inquiries and connect with available resources.

  • Case Management - Assesses needs and coordinates services for older adults in the Elderly Services Program.

  • TakeCare - An educator/case manager works with eligible TANF (Temporary Assistance for Needy Families) recipients and the job worker who receives a work exception due to caring for a family member.

Provided to individuals who have been found by the Probate Court to be unable to make appropriate decisions for themselves.

  • Volunteer Guardians receive ongoing direction and support from LifeSpan staff and have completed training and background checks.

  • Guardians develop a personal relationship with the client.

  • Guardians advocate for the client's needs.

  • Financial services are also available from staff guardians.

Family Connection

When you want to be there...and can't.

Professional staff will:

  • Develop a personalized care plan to help your family member live independently with dignity, comfort and safety.

  • Arrange for needed services such as nursing care, homemaking / personal care, home delivered meals and transportation.

  • Monitor the well-being of your family member and communicate with you on a regular basis.

 

Representative Payee

 

 

Services are provided to individuals for whom it has been determined, by a medical officer, cannot manage their own funds.

  • Staff personnel have over 18 years experience in payeeship services.

  • Develop a workable budget to ensure basic needs are provided.

  • Receive income and pay all bills.

  • Staff work to develop a trustworthy and close relationship with clients.

  • Serve as liaison for Social Security and client.
